Exam Agreement Forms

Candidates will be required to review and sign an Agreement that is part of their exam. After check-in, candidates in the test area, you will be bound by the Agreement and Rules. Candidates will be asked to leave the test center immediately if they don’t agree to be bound by the Agreement and these Rules. If candidate needs additional details about any of the rules or their expected conduct, please ask the Test Administrator/Proctor.
