After completing your report, please contact your phone service provider, as noted on your telephone bill, with the report number you receive at the end of this process. Follow your phone service provider's instructions and keep track of when calls occur and the number they are from (if you have caller ID). This reporting form is for use in situations in which no suspect information is present. (Examples: Your car was parked unattended and an unknown driver/vehicle hit your car and didn’t leave a note or a vehicle hit your car and then fled without stopping to exchange information or before you could get a license plate.)

If you are the victim of identity theft or fraud please get and keep all papers, receipts, cancelled checks, mail from companies, etc. related to this crime. We may contact you to obtain copies of these items if needed. It is recommended that you have copies of all related documents for your records.
If you are the victim of identity theft or fraud institutions such as your bank, may ask for a police report number. A report number will be generated at the end of this online police report process.
Helpful Tips:
- Protect your social security number and only give it out when required by law.
- No bank, auction site or other business should send you an email asking you to enter credit card information, passwords, social security numbers or other personal information. Often these fake emails will contain links to pages that look real but are not. If you suspect you are being directed to a fake site please go to the actual home web site of the company and inform them of what has happened.
- Avoid mail theft by obtaining a locked mailbox and dropping off outgoing mail at the local post office or postal mailbox.
- Do not give out your personal information or credit card number over the phone unless you initiated the call.
- Shred credit card offers and bank statements before you throw them away if you do not plan on using them.

Whenever possible the Department of Police Services will assist the university community in retrieving lost property, even though no crime is involved.
If you find someone else's lost property, please bring the item to the Department of Police Services or report it promptly to the Department of Police Services at 818-677-2111. An officer will secure the property, try to identify the owner and return the property to them when possible.
There are two separate methods for reporting lost property:
You may file an online lost property police report if your lost property meets one or more of the following criteria:
- The lost item you are reporting is state-owned property.
- Or you plan to have your lost item replaced and must submit a police report to your insurance company.
- Or you have a serial number or owner-applied serial number for your lost item.
- Or you are reporting an item valued at $950.00 or more with or without a serial number.
For all other lost personal property reports that do not meet one or more of the above listed criteria use the Lost and Found Reporting System.
You may access this system at any time, day or night to do the following:
- File an report online for your lost personal property.
For your convenience, the Lost and Found Coordinator will use your contact information to notify you when your property has been recovered, so you may retrieve it from the Lost and Found.
If you know the general area where you lost the property, such as in the Library, Bookstore or the University Student Union outlets, don't forget to call the store or main offices to check if your property has been turned into them. Stores and offices on campus will frequently hold Lost/Found Property for a period of time prior to forwarding the item to Department of Police Services for safekeeping.
