The Authorization to Use Privately Owned Vehicles on State Business (STD261) must be filled out the first time this person drives on a field trip. The form is to be authorized by the Department Head/Chair and filed in the department. This form must be renewed annually.

The Driver must identify those students who will be in their vehicle. These individuals must be listed in the Student Authorization Privately Owned Vehicle Form, which must be completed by the driver.

Be sure insurance information is complete before faculty countersigns. This form is to be authorized by the Department Head/Chair and filed in the department.
