HR COVID-19 Medical Provider Employee Testing Resources

Employees may access free test kits through various Los Angeles County locations - COVID-19: Testing - Health Services Los Angeles County (lacounty.gov) as well as through their medical insurance provider.  Insurance providers are to provide eight test kits per month.  We encourage employees to reach out to their insurance provider on the process. 

Additionally, CSUN will provide rapid tests to employees who are identified as a close contact by the university or need to test in order to leave isolation/quarantine. Please complete the CSUN Rapid Test Kit Request form to request a rapid test kit. These kits will be mailed and should arrive in 3-5 business days. 

If you test positive for COVID-19 or are a close contact, please submit a self-report using the Faculty - Staff Reporting Form.

Visit COVIDtests.gov for information on free tests that may be available through government services.

Anthem has a limited number of at-home diagnostic test kits available for certain members to order online. Log in using the SydneySM Health mobile app or at anthem.com/ca. If you’re eligible to order an at-home test, you’ll see the option to place an order. If you don’t see that option, you’re ineligible but you still have other testing options available.

At-home diagnostic tests can be purchased over the counter, online, at a local pharmacy, or from a big-box store.

You can still use in-person diagnostic COVID-19 testing sites. These may include your doctor’s office, a pharmacy or health clinic, or an urgent care center. Use our COVID-19 Test Site Finder to find a testing location close to you.

Check with your pharmacy to see if they can bill the vendor through their pharmacy system. You can then get an OTC at-home COVID-19 test at no cost. You can also purchase an OTC FDA-approved, EUA at-home COVID-19 test and be reimbursed by Health Net for the cost of the test. Please complete and submit a Health Net medical claim form.You can receive COVID-19 testing at no cost from an in-network or out-of-network provider. You can get treatment for covered services based on your plan benefits. You should refer to your Policy or Plan Contract and Evidence of Coverage for provider network and cost-share information.

Anthem's at-home test kit program is only available to individual members and members of specific employer groups for a limited time. Log in to find out whether you’re eligible. If you’re eligible, you’ll be able to place an order. If you are ineligible, you can utilize the test site finder for available options close to you.

Anthem's test kit is available at no charge for diagnostic testing only. This test is for members who have symptoms of COVID-19 or who have been exposed to someone with confirmed or suspected COVID-19. A doctor’s order, while normally required for testing to be covered under your plan, is not required when ordering a test through this limited program. This Rapid Diagnostic Test for the Detection of SARS-CoV-2 Antigen is an OTC Home Test for use under an FDA Emergency Use Authorization (EUA) for in vitro diagnostic use only. Is it not being made available under FDA authorization for screening purposes.

The test typically arrives in two business days or less and provides results in 10 minutes.
