There are two parts to this assignment, 1) you will create an online survey to collect data and 2) generate a graph using the data.
Some helpful hints:
A use for 'Length' is for example when you want to rate items, but not only on a scale of 1-5, but you might want 1-?. If you want more than 5, place the desire number in the 'Length' field.
'Required' dictates whether the survey question has to be entered prior to submitting the survey. If you select 'Yes,' the individual has to answer the question prior to submitting.
When you are finished with the questions, you will need to go to the 'Management Interface' and activate your survey. Once you activate your survey, you can not make any changes to your survey, so make sure your survey appears the way you want it to look prior to activating.
If you activate your survey and find some mistakes, you can copy your survey from the 'Management Interface' and make the needed changes.
I. Creating an Online Survey
You will need to go to http://media.csun.edu/~webteach/phpesp and a log in screen will appear, you will log in using your userid and password. The same one used in accessing WebCT and your U-Drive.
Once you log in, you will come to a screen that looks like the following
You will want to click on the 'Create a New Survey' link just below your username.
Fill in the required text areas and click on 'Continue.'
Prior to entering questions, lets review some items...
'Field Name' is what you would like to call the question, for example you could simply list them in numeric or alphabetical order. You could even give them a name. This is used later to view the questions and is not too terrible important
Under 'Type' there is a drop down menu and when you click on it, you will notice a number of choices of how you want the questions to look. I have placed some of the different types of questions. Feel free to use these questions, or come up with your own. You need to have at least 9 questions and the following 7 different types of questions (text field, yes/no, rate, essay, radio button, check box and dropdown box).
Text Field: Name (first then last)
Text Field: Date (04/06/05)
Yes/No: Do you think this technology class is useful?
Rate (scale 1..5): Rate the following assignments (1=bad
assignment, 5=excellent assignment)
Essay Box: Which assignment(s) were useful, and why?
Essay Box: Which assignment(s) were NOT useful, and why?
Radio Button: How many years have you been teaching?
Check Boxes: Which school district(s) have you worked for?
Dropdown Boxes: Which department have you declared?
Once you click on 'Continue,' the next page should be titled 'Survey Design' with the multiple tabs. The 'Questions' tab should be selected and you are now ready to start entering questions. (the following picture is an example of the third question that I am entering)
After entering a question, you will want to click on 'New Field.' that will bring up the next question.
After entering all of your questions, click on 'Continue' and the 'Order' tab should appear. Here, you can re-order the numbers and make changes to questions. If your questions look ok, click on 'Continue.' Now the 'Preview' tab should appear and you can see what the actual questions will look like. If you find any mistakes, click on the 'Order' tab and make your changes. Once you feel that your survey looks good and you want to start it, click on the 'Finish' tab. There, you will see where your URL will be. Print out this page, so you do not forget the URL. On the bottom of the page, it should state 'Go back to Management Interface'
Now that you are on the 'Management Interface' page, select 'Change the status of a Survey' so you can activate the survey.
Find your survey and select 'Activate.' Now go to your URL and see your online survey.

II. Generate a graph using the data
Creating a graph is OPTIONAL, and the some surveys are difficult to create graphs.
Whether you use the data from my survey or use the data from your own, you will create a graph with at least 4 categories, (ex. from my survey, Do you think this technology class is useful, How many years have you been teaching, Rating the different assignments)
Feel free to use my questions, or come up with your own. You need to have at least 9 questions and the following 7 different types of questions (text field, yes/no, rate, essay, radio button, check box and dropdown box).
Now that the survey is online and working, you will want to link it to your portfolio Web page titled 'Survey.' You will also need to create a link titled 'graph,' for your survey graph.
