53rd Annual Staff Service Awards: Is It Your Time To Be Recognized?
CSUN will celebrate the 53rd Annual Staff Service and Recognition of Excellence Awards Event Monday, April 22, 2019. This event recognizes employees who have achieved milestones in five-year increments of dedicated service to the University. At this year's event, we expect to honor approximately 306 employees recognized for collectively providing more than 4,360 years of service to the University!

Fall 2019 Fee Waiver Priority Enrollment Period | February 25 thru April 26
The CSU Employee Fee Waiver and Reduction Program provides for the waiver or reduction of tuition and certain fees for eligible employees. In addition, an eligible CSU employee may transfer their fee waiver benefit to a spouse, domestic partner, or eligible dependent child. CSUN employees who want to complete a bachelor’s or graduate degree, enhance job skills, or even prepare for a future career, are encouraged to participate in the program. Eligible employees or their eligible dependent may have tuition and fees for a maximum of two (2) courses or six (6) units, whichever is greater, waived/reduced each semester.
The CSU Fee Waiver module will be available for Fall 2019 applications beginning February 25, 2019. In order to ensure timely processing, please submit your application no later than April 26, 2019.
Partnering with Admissions & Records and Student Accounting, we will be offering a workshop on the Fee Waiver and Reduction Program on Monday, March 25, 2019 from 12 to 1 p.m. in the HR Training Room, Oviatt Library, Room 16. Join us and get your questions answered!

Part-Time Faculty Online Benefits Overview
If you are a new part-time faculty member or returning faculty and are benefits eligible* for the Spring semester, we invite you to click here and view information on plan options, eligibility, and how to enroll. Benefits Packets are also available in the Office of Human Resources, UN 165. Remember, you have 60 days from the effective date of your initial hire to enroll in benefits.
*To be eligible for benefits, part-time faculty must have an Academic Year (AY) classification and have an appointment of at least six units (.4 time base) for one semester.
Faculty currently enrolled in Medical/Dental/Vision benefits or FlexCash who fall below six units should notify Benefits Administration regarding continuation of coverage. COBRA coverage may be available. For information on COBRA please visit http://www.csun.edu/benefits/cobra.
Did You Receive Your 2018 W-2 Tax Form?
The State Controller’s Office (SCO) will mail all employee 2018 Form W-2 Wage and Tax Statement by no later than January 31, 2019. These W-2s will be mailed to the employee’s home address listed on file in SOLAR HR as of close of business on December 17, 2018. Employees who do not receive his/her W-2 by mid-February should contact Payroll Administration at (818)677-7550 to inquire if it has been returned to the campus by the SCO.
For W-2s that are not returned to the campus, or if a duplicate copy is needed, submit a request for a duplicate copy using Standard Form 436 (Request for Duplicate Wage and Tax Statement.) Additional information and the duplicate W-2 request form are available through the SCO at https://www.sco.ca.gov/ppsd_empinfo_dpw-2.html. Please note that there is an $8.50 processing fee per tax year for duplicate W-2s; money/cashier’s check only (no personal checks) or payroll deduction (only active employees eligible.) All requests should be sent directly to the SCO at the following address:

Employer-Provided Health Insurance Offer and Coverage Statements (1095-B and 1095-C) for Tax Year 2018
Under the Affordable Care Act (ACA), the State of California is required to file annual reports with the Internal Revenue Service (IRS) and furnish a statement to full-time employees with information about the health coverage offered to the employee and their dependent children, using IRS Form 1095-C. Health coverage providers are also required to file annual reports with the IRS and furnish a statement to individuals with information about those who had minimum essential coverage for at least one day during the preceding calendar year, using IRS Form 1095-B.
By January 31, the State of California, via the State Controller’s Office, will issue a Form 1095-C to its full-time employees with information about the health coverage offered, if any, to the employee and their dependent children. The state’s health coverage providers will also issue a Form 1095-B to employees who were enrolled in state-sponsored health coverage for at least one day during the preceding calendar year.

2019 Health Plan Rates - Check Your January Pay Warrant
The 2019 Health Plan Rates are available on the Benefits Administration webpage. Please remember to check your January pay warrant for your premium deduction and report any discrepancies to your Benefits Representative. If you made any changes during the Fall 2018 Open Enrollment Period, your new plan(s) and premium should be reflected on your January 2019 pay warrant. In addition, you should have received written confirmation from your health plan and identification cards (as applicable).
As a reminder, you can view your benefit elections under the Benefits Summary and the Dependent and Beneficiary Coverage Summary in the portal. If you submitted forms to make changes as part of Open Enrollment, change the date to 1/1/2019 and select Go.

Resolve to Save More for Retirement? 2019 Information for 403(b), 401(k), and 457 (b) Retirement Plan
Is it time for a Financial Wellness Check-up? The CSU offers you additional ways to save for your future.
- For the 2019 tax year, the contribution limits are increasing from $18,500 to $19,000 to a 403(b) or 401(k) plan, subject to the 415(c) limit, AND $19,000 to a governmental 457(b) plan, for a total contribution of up to $38,000.

Effective 1/1/2019, the California State University (CSU) 403(b) Tax Sheltered Annuity (TSA) Program will be referred to as the California State University (CSU) 403(b) Supplemental Retirement Plan (SRP). Healthcare Reimbursement Account (HCRA) and Dependent Care Reimbursement Account (DCRA) Claims
The CSU provides for the 2 ½-month grace period extension allowed by the U.S. Department of Treasury for flexible spending plans. As a result, current participants (as of December 2018) with balances in the Health Care Reimbursement Account Plan (HCRA) and/or the Dependent Care Reimbursement Account Plan (DCRA) as of December 31, 2018 will be allowed to utilize remaining 2018 balances toward reimbursement of eligible medical and/or dependent care expenses incurred from January 1, 2019 through March 15, 2019. Any balance remaining in either a 2018 HCRA or 2018 DCRA account after March 15, 2019 will be forfeited.
All HCRA/DCRA claims for services provided 1/1/2019 – 3/15/2019 will automatically be processed against a remaining 2018 balance if filed by June 30, 2019. If your claim exceeds the available funds from 2018, any excess will be automatically applied to your 2019 plan.

CalPERS Dependent Re-Verification Requirement - Is Your Birthday In May?
Dependent Re-Verification (DRV) is the process of re-verifying the eligibility of your spouse, domestic partner, children, stepchildren, and domestic partner children (family members) enrolled for state health and dental benefits. Government Code section 22843.1 mandates re-verifying the eligibility of your family members. Government Code section 22959 authorizes extending this review to your family members enrolled for dental benefits.
The eligibility of your family members will be re-verified once every three years. The re-verification period is based on your birth month. Employees born during the following months are required to re-verify family members during 2019: February, May, August and November.
For those employees with birthdays in May, CalPERS, will begin mailing you information in February explaining the process and the documentation required to maintain health coverage for your dependents. Please read and ensure Human Resources has your documents prior to May 1, 2019 in order for your dependent(s) to continue receiving health coverage and avoid cancellation on 06/1/2019.
